To fill out this application, you will need to know the following:

Minimum age requirement: 21 years of age
- Social Security Number or Social Insurance Number
- Current driver license number and driver license history
- Employment history for the last five years (If you have operated a commercial motor vehicle within the last ten years, you will need to provide employment history for ten years)
- History of all traffic convictions and accidents for the last five years, including month/year and complete details. (Employment will be determined depending on convictions that include reckless driving, suspensions, excessive speeding, etc)
- All Criminal history
- History of any DUII conviction(s) (No convictions within the last three years allowed to be considered for employment)
