Print and complete a separate copy of page five (Inquiry to Past Employer) for each employer within the last five years. Completing this entire application is the first step in our screening process.

You'll need the following to fill out this application:

  • Minimum age requirement: 21 years of age.
  • Social Security Number or Social Insurance Number.
  • Current driver license number and driver license history.
  • Employment history for the last five years. If you have operated a commercial motor vehicle within the last ten years, you will need to provide employment history for ten years.
  • History of all traffic convictions and accidents for the last five years, including month/year and complete details. Employment will be determined depending on convictions that include reckless driving, suspensions, excessive speeding, etc.
  • All criminal history.
  • History of any DUI/DUII conviction(s). Convictions within the last three years will remove applications from consideration for employment.
