To fill out this application, you will need to know the following:

• Social Security Number or Social Insurance Number
• Current driver license number and driver license history
• Employment history for three years (if you have operated a commercial motor vehicle within the last ten years, you will need to provide employment history for ten years)
• History of traffic convictions and accidents from the last five years (including month/year and complete details)
• Criminal history
• History of any DUI conviction, reckless driving conviction and license suspension
